Findings: The Alleged Victim (AV) is a known fall risk and has suffered previous falls. On or about July 25, 2023, AV was found with a small cut and a bump on his/her head. No fall interventions were on AV's service plans, only in progress notes. Staff could not recall how often they were to check on AV. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
