- An unknown Alleged Perpetrator 2 (AP2) neglected Alleged Victim (AV) by failing to provide the basic care and services necessary to keep AV safe when AP2 provided AV with an illicit substance, resulting in harm to AV. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
