Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls. On or about September 5, 2023, AV suffered a fall causing a skin tear to his/her right wrist. The facility failed to properly care plan for AV, with appropriate interventions to reduce his/her risk of falls. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
