- Findings
- Alleged Perpetrator 2 (AP2) grabbed both of Alleged Victim's (AV) wrists hard and cussed and yelled at AV while AP2 was assisting AV with his/her Activities of Daily Living. AP2 is responsible for physical abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
