- Findings
- Under prior ownership: The facility failed to follow the AV's care plan for safety checks and to have supervision according to the AV's needs. The failure resulted in AV experiencing an unwitness fall resulting in hospitalization, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
