Findings: The Alleged Victim (AV) suffered 7 falls with injury over a two month time span. AV's care plan states that he/she is independent with mobility and transfers, however, all falls occurred when AV was trying to go to the bathroom or transfer to his/her chair. No new interventions were put into place after AV's falls to address the falls while transferring. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
