Findings: According to documentation, the AP2 failed to follow the Alleged Victim’s care plan and facility protocol to not deliver medication directly to AV. The failure resulted AV experiencing an overdose in medication and was sent the hospital and later passed away. AP2’s actions are considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
