Findings: The facility failed to follow Witness 1's (W1) care plan to redirect residents away from his/her room as an intervention to mitigate the risk of resident-resident altercations. The failure resulted in the Alleged Victim (AV) wandering near W1's room causing a physical altercation and AV was pushed to the ground causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
