Findings: The Alleged Victim (AV) had a history of falls. On or about December 6, 2023, AV suffered a fall in his/her room, which resulted in a bruise and pain to his/her head. The facility failed to properly care plan as it relates to AV's history of falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
