Findings: The facility failed to provide appropriate services to the Alleged Victim (AV) according to his/her need for toileting assistance. Staff admitted to not being able to provide assistance due to a shortage of staff. The failure resulted in AV experiencing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
