Findings: The Alleged Victim's (AV) service plan was not reflective of interventions to address specific behaviors of AV taking food from other residents or staff during dining service. Instructions outlined in AV's service plan to redirect AV were not attempted during the meal observation on 12/12/23. The facility's failure to properly care plan according to AV's needs escalated AV's behaviors and caused a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
