Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) history of repeated falls. On or about August 17, 2023, the AV suffered a fall in their restroom. The facility staff found the AV in their restroom and immediately called emergency services. The AV was transported to the hospital and was diagnosed with a broken collar bone, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
