Findings: According to the documentation, the facility failed to follow the care plan for Witness 1 (W1) when they are expressing aggressive behavior. On or about December 21, 2023, W1 was observed by staff approaching the Alleged Victim (AV) aggressively. Facility staff did not intervene, but instead turned away and W1 hit the AV in the throat. There was no documented evidence the facility attempted interventions for W1 when they are exhibiting aggressive behavior to other residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
