Findings: According to the documentation, the facility failed to provide a safe medication administration system for Witness 1 (W1). From approximately November 29, 2023, through December 09, 2023, the W1 was on a trial period for scheduled psychotropic medication. After this trial period ended, on or about December 17, 2023, W1 engaged in a physical altercation with the Alleged Victim (AV). Staff had to separate W1 and the AV throughout the entire day. The scheduled psychotropic for W1 was not reordered until approximately January 11, 2024, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
