Findings: The facility failed to provide a safe environment. On or about January 8, 2024, according to documentation the Alleged Victim (AV) sustained fractured ribs from an unknown origin and was sent to the Emergency Department for treatment. The failure resulted in AV experiencing pain and unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
