Findings: Alleged Victim (AV) has a known history of falls. Between November 21, 2023, and December 6, 2023, facility care staff failed to review several temporary care plans which included fall interventions. During that same time period, AV suffered several falls and was injured. On or about December 6, 2023, a Home Health Physical Therapist left the following written instructions for staff, “Instructions: Requires constant supervision and assistance for safety due to high fall risk.” The facility disregarded the Physical Therapist’s December 6, 2023, written instructions. The facility failed to provide a safe environment for AV, resulting in falls to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
