Findings: Witness 1’s (W1’s) interventions are to ensure room level supervision when Alleged Victim (AV) and W1 are in the same area. On or about February 22, 2024, while AV was lying on the floor, W1 struck AV about two to three times on the back of AV’s head. During the altercation staff did not have room-level supervision on AV or W1. AV and W1 have a history of resident altercations with each other. The facility failed to follow the care plan regarding room level supervision, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00218 $500.00 fine assessed
