Findings: The facility failed to follow the Alleged Victim's (AV) care plan to mitigate the risk of resident to resident altercations. According to AV's service plan, staff are to provide AV a doll or stroller to keep his/her hands busy to avoid touching other residents, assist AV to a smaller environment for meals and keep him/her safe distance from Witness 1 (W1) due to prior altercations. On or about 02/17/2024, AV walked passed W1 and grabbed his/her meal plate, resulting in a physical altercation. The facility's failure to follow AV's care plan and provide a safe environment, is a violation of resident rights, is considered neglect of care and constitutes abuse.
