Findings: Alleged Victim (AV) is known to be a high fall risk. AV has sustained two falls on or about February 1st and 3rd, 2024. On or about February 6, 2024, AV had an injury fall. AV hit his/her head and was sent to the hospital and was sent back to be monitored. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
