Findings: Witness 1 (W1) has a known history of extremely violent behaviors towards staff and residents. On or about February 9, 2024, Alleged Victim (AV) was walking through the dining room with staff. A resident began to yell at AV, which caused W1 to begin to slap AV in the face. W1 has had multiple documented assaults on staff and residents, however, the facility has failed to care plan around W1’s known behaviors of aggression, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00241 $500.00 fine assessed
