Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about February 10, 2024, AV was crying while walking towards staff down the hall and reported that Witness 1 (W1) came into AV’s room and attacked AV by grabbing AV’s breast really hard. The facility failed to provide a safe environment for AV, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
