Findings: Alleged Victim (AV) has a known history of grabbing other resident’s walkers and personal items, causing resident-to-resident altercations. On or about February 5, 2024, AV approached Witness 1 (W1) while leaving the dining room. W1 pushed AV away from him/her, and AV grabbed W1 causing them both to fall. As a result of this altercation, AV fell and hit his/her head on the fire door, W1 fell and fractured his/her hip. The facility failed to provide a safe environment for AV and W1, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00214 $1500.00 fine assessed
