Findings: Witness 1’s (W1’s) interventions are to ensure room level supervision when Alleged Victim (AV) and W1 are in the same area. On or about February 5, 2024, after dinner, W1 approached AV leaving the dining room. AV pushed W1 away from him/her, and W1 grabbed AV causing them both to fall. W1 has a history of grabbing other residents’ walkers and personal items, causing resident to resident altercation. During the altercation interventions were not being followed. The facility failed to follow the care plan regarding room level supervision, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00216 $500.00 fine assessed
