Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) known fall risk. On or about March 16, 2024, the AV sustained a fall in their bathroom and pulled their call light for assistance. The facility staff responded to the call light approximately two hours after the AV initially pulled the call cord in their bathroom. After evaluation of the resident and injuries, the AV was sent to the hospital for assessment where they were diagnosed with a neck fracture, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
