- Findings
- According to documentation, the AP2 used physical force when s/he pushed AV into the dining room table in a rough manner. AP2’s actions are considered physical abuse through the use of force that may cause injury, pain or impairment. The facility failed to protect AV from abuse which is a violation of Oregon Administrative Rules.
