Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) recent falls. On or about February 19, 2024, the AV was found on the floor and transported to the hospital for assessment. The AV suffered a large hematoma and was anemic, which required two blood transfers, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
