Findings: Witness 1 (W1) is known to wonder into other resident’s rooms. On or about April 2, 2024, staff heard yelling and went down the hall. When they entered Alleged Victim’s (AV’s) room, they saw AV holding W1 by the hair and W1 swinging his/her arms and hitting AV. This has not been the only altercation W1 has been involved in. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00829 $500.00 fine assessed
