Findings: According to the documentation, the facility to provide a safe medication administration system for the Alleged Victim. After review of training documentation, it was determined the facility failed to train the Alleged Perpetrator 2 (AP2) to safely pass medication before passing medication. The failure resulted in the AP2 to administer the wrong dose of behavioral medication causing risk of increased behaviors. AP2’s failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse. The facilities failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
