Findings: Alleged Victim (AV) is known to be an exit seeking resident. AV has also previously eloped from the facility when movers were bringing furniture for a new resident. On or about March 23, 2024, AV walked out a side emergency exit door while a mover was moving furniture into the facility. There have been no new interventions put in place to stop another elopement from occurring to AV. The facility failed to properly plan care around AV exit seeking behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
