Findings: The facility failed to implement interventions and provide appropriate supervision of Witness 1 (W1) according to his/her behaviors. On or about April 19, 2024, W1 was found in the Alleged Victim's (AV) room on two occasions, the second time resulted in AV being inappropriately touched by W1, causing emotional distress and a loss of personal dignity. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
