Findings: The Alleged Victim (AV) relies on the facility for his/her care. Fall interventions documented in AV's care plan included to offer AV assistance to the bathroom at 8:00 pm, use of a nightlight to help light the way for nighttime bathroom needs and placing a pool noodle under the sheet of the bed to help AV establish boundaries of the bed. According to an investigation, on or about April 28, 2024, AV fell near their bed, resulting in a bruise and swelling to their face. The fall interventions had not been provided prior to the fall. The facility failed to follow the AV's care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
