Findings: According to the documentation, the facility failed to provide a safe environment to ensure the facility was secured and prevented the Alleged Victim (AV) from eloping. On or about June 10, 2024, the AV managed to exit the secured facility without the staff knowing. At approximately 8:15 pm, local law enforcement found the AV in a neighboring apartment complex and notified the facility of the elopement. The facility failed to provide a safe secure environment which is a violation of resident rights, is considered neglect of care and constitutes abuse.
