Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) known fall history. On or about June 30, 2024, the AV was found outside of another resident’s room on the floor. The AV was complaining of shoulder and neck pain and was sent to the hospital for evaluation. At the hospital, the AV was diagnosed with a fracture in their cervical spine. Upon further investigation, the facility did not ensure the AV had their mobility device with them at the time of the fall, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
