Findings: Alleged Victim (AV) has a history of scratching and aggression towards facility care staff. AV has verbal outbursts and screams and yells especially if AV has to wait for something AV wants. AV requires assistance with transferring. AV was screaming in the common area for a snack; facility care staff tried to calm AV down, but AV kept screaming. AP2 couldn’t calm AV so AP2 was transferring AV to AV’s wheelchair and assisting AV to AV’s room. AP2 went to grab AV’s arms to transfer AV, when AV scratched AP2 in the left arm and left eye. AP2 slapped AV’s hand away. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility’s failure is a violation of Oregon Administrative Rules.
