Findings: Witness 1 (W1) has a known history of resident-to-resident altercations due to W1 wandering into other residents’ rooms. On or about July 11, 2024, W1 entered AV’s room, AV grabbed W1 by W1’s hair and pulled W1 out of AV’s room. W1 then pushed AV onto the ground. The facility failed to provide a safe environment for AV, which puts AV in risk of harm and is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00843 $375.00 fine assessed
