- Findings
- Alleged Victim (AV) has a known history of resident-to-resident altercations due to AV wandering into tother residents’ rooms. On or about July 11, 2024, AV entered Witness 1’s (W1’s) room, W1 grabbed AV by AV’s hair and pulled AV out of W1’s room. AV then pushed W1 onto the ground. The facility failed to provide a safe environment for AV, which puts AV in risk of harm and is a violation of resident rights, is neglect of care and constitutes abuse.
