Findings: Based on witness statements, investigator observations, and documentary evidence collected during Adult Protective Services (APS) Investigation #00344238, the APS Investigator determined that on or about July 23, 2024, the facility failed to provide appropriate services in accordance to the Alleged Victim’s (AV) care needs, specifically related to wound care following AV's return to the facility with leg wraps. As a result, AV’s leg dressings were not changed between July 9, 2024, and July 22, 2024, leading to a worsening of AV’s condition, causing further unreasonable discomfort. The facility's failure to provide timely and appropriate wound care is a violation of resident rights, is considered neglect of care and constitutes abuse. Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no abuse occurred by AP2.
