Findings: The Alleged Victim (AV) has care needs that include a specific dental diagnosis that requires daily, consistent care. AV's care plan regarding the oral care was vague and missing two of the steps necessary to ensure AV's oral hygiene. AV was found frequently to have remains of food around his/her gum line, heavy plaque buildup and stained teeth. The facility's failure to ensure proper oral hygiene placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
