Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 9, 2024, Alleged Perpetrator 2 (AP2) was assisting AV with toileting after a bowel movement, when AP2 told AV that they were "nasty". AV became upset and asked AP2 to leave. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility failed to protect AV from verbal/emotional abuse which is a violation of Oregon Administrative Rules.
