Findings: Witness 1 (W1) has a history of being the aggressor in resident-to-resident altercations. At the time of the altercation, W1 interventions were to have eye level supervision. On or about September 6, 2024, while in the living room, W1 punched Alleged Victim (AV) in the face and body multiple times. Staff were not in the living room when the altercation occurred. The facility failed to follow W1’s care plan, by failing to have eye level supervision, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00110 $500.00 fine assessed
