Findings: Witness 1 (W1) has a history of resident-to-resident altercations, with previous altercations on or about August 21st, and September 10, 2024. On or about October 23, 2024, W1 approached a group of residents in the dining room. W1 grabbed one resident by the shoulder before punching the resident in the back of the head. W1 also grabbed Alleged Victim (AV) by the wrists during this altercation. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00166 $500.00 fine assessed
