Findings: Witness 1 (W1) has a known history of resident-to-resident altercations. Previous altercations were on August 21 and September 10, 2024. On or about October 23, 2024, W1 approached a group of residents in the dining room. W1 grabbed Alleged Victim (AV) by the shoulder before punching AV in the back of the head. After the altercations, there was no new interventions placed to prevent further altercations. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-00250 $500.00 fine assessed
