Findings: The Alleged Victim (AV) relies on the facility to meet his/her daily needs. According to an investigation, on or about October 26, 2024, the Alleged Perpetrator 2 (AP2) wrongfully physically restrained the AV when AP2 held AV's hands during personal care when AV was refusing care, resulting in AV experiencing anger and unreasonable discomfort. AP2's actions are considered neglect and constitutes abuse. The facility failed to assure resident's rights, which violates Oregon Administrative Rules.
