Findings: Alleged Victim (AV) relies on the facility for his/her care needs. On or about December 6, 2024, AV was found to have a dark purple bruise the size of a baseball on AV's left arm. The bruise was in the shape of a hand. AV said that it was caused by a staff member, but AV could not remember which staff member or how. It is believed that an unknown Alleged Perpetrator 2 (AP2) caused the injury to AV. AP2 failed to protect AV from physical abuse, which is neglect of care and constitutes abuse. The facility failed to protect AV from physical abuse, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-01069 $250.00 fine assessed
