Findings: Witness 1’s (W1's) service plan states that W1 is not easily angered and does not require intervention to prevent harm to others. W1's service plan also says that W1 has a history of agitation or anxiety triggered by other residents not doing what W1 thinks they should be doing. W1 has a history of verbal cruelty to other residents and resident to resident altercations. On or about June 19, 2024, W1 smacked Alleged Victim (AV). AV attempted to retaliate but was unable to. AV said, "that crazy bitch hit me twice." W1 punched AV twice in the stomach. Despite W1 having a documented history of being aggressive the facility did not put interventions in place for W1's behaviors until after this altercation, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP25-01058 $500.00 fine assessed
