Findings: The Alleged Victim (AV) and Witness 1 (W1) both rely on the facility for their care and have a history of physical altercations. According to an investigation, on or about December 11, 2024, W1 struck AV with a closed fist and palm. AV experienced emotional distress and was fearful of W1. The facility failed to provide a safe environment for AV, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
