- Findings
- Alleged Victim (AV) requires a 2-3 person assist for all transfers or a Hoyer lift. The facility does not use Hoyer lifts. AV is unable to assist with transfers. On or about December 19, 2024, Staff were helping AV transfer from the toilet to AV's wheelchair when the plastic lock on AV's gait belt broke. Staff assisted AV to the floor. AV was helped back off the floor and into AV's wheelchair. AV complained of pain in AV's right knee after this. AV had bruising to AV's right upper bicep. On or about December 27, 2024, staff were transferring AV from AV's wheelchair to AV's bed when AV got a laceration approximately 2 inches long on AV's right knee. AV had to be transported by EMS to the hospital and required 3 stitches in AV's right knee. On or about January 2, 2025, AV bandage on right knee was dated December 31, 2024. The facility failed to provide a safe environment, maintain care equipment for resident scheduled and unscheduled needs and failed to have enough staff to provide for AV's transfers resulting in injury.
