Findings: The Alleged Victim (AV) is dependent on facility staff to meet his/her care needs and has a history of falls. According to an investigation, on or about January 26, 2025, AV fell out of bed, was experiencing pain and was transported to the hospital. AV was then diagnosed with a hip fracture. The facility had received an order for half bed rails on AV's bed for mobility on December 23, 2024. The bed rails were in the lowered position when AV was last seen by staff prior to the fall. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
