Findings: The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. According to an investigation, on or about February 24, 2025, the Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) subjected AV to involuntary seclusion when AP2 pulled AV's wheelchair backwards into AV's room and AP3 held AV's hands to assist AP2 with getting AV into their room and shutting the door. These actions resulted in AV feeling upset and requiring transport to the hospital for verbal and physical aggression. AP2 and AP3's actions are a violation of resident's rights, are considered neglect of care and constitutes involuntary seclusion and abuse. The facility failed to protect AV from abuse, which is a violation of Oregon Administrative Rules.
