Findings: The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. According to an investigation, on or about February 24, 2025, the Alleged Perpetrator 2 (AP2) and AV were involved in a verbal and physical altercation after AP2 forcefully removed AV from the common area. AP2 physically abused AV when AP2 grabbed AV's forearms, resulting in a skin tear to AV's hand and requiring transport to the hospital for aggression. AP2's actions are a violation of resident's rights, are considered neglect of care and constitutes physical abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
